University Medical Partners (UMP), a partner of Stanford Medicine, is seeking a full-time experienced BC/BE Palliative Care or Geriatric Medicine Physician Medical Director.  Position is based in Newark, CA with travel to various community-based clinics, acute care facilities, and/or patient homes throughout the Bay Area.

Position Details:
The Medical Director, Quality Improvement and Care Coordination Program is the physician leader for providing comprehensive care coordination and Palliative Medicine consultations for the high-risk high-needs patients managed by the UHA Accountable Care Division – Care Management Program (ACD-CM) and the UHA/UMP Community-Based Palliative Medicine Department (PMNWK)
  • Coordinate comprehensive care for patients identified as eligible for the ACD-CM and/or the PMNWK program to deliver high-quality outcomes and experience
  • Lead a team that may include physicians, NP/PA’s, nurses, social workers, pharmacists, care coordinators and others. This includes providing oversight, education, and team building. Responsible for hiring, training, and ongoing review of team members.
  • Review referred patients to determine who would benefit from the program
  • Monitor enrollment to maintain a census of the maximum number of patients who would benefit from the ACD-CM and/or the PMNWK program
  • Use results provided by UHA and health plans to adapt clinical programs to improve the care provided to members
  • Participate with UHA Accountable Care and health plans to determine the optimum support for patients considering care coordination and/or Palliative Medicine consultations options available in communities, in plans, or other parts of Stanford Health Care
  • Specific responsibility for the coordination of medical care and support services rendered by members of the ACD-CM and the PMNWK team
  • Monitor and report on outcomes for the patients cared for in ACD-CM and by PMNWK
  • Collaborate with the Administrative team to ensure proper functioning of the day-to-day operations
Clinical Care Delivery & Supervision (estimated 80% of time)
  • Provide direct patient care (at least 50% of the time) and oversight of clinical team and resources caring for current and prospective ACD-CM and Palliative Medicine patients
Leadership (estimated 10% of time)
  • Serve as a clinical lead in Accountable Care to achieve improved outcomes for managed populations (e.g., reduced readmissions, reduced avoidable admissions)
    • Participate in the development of strategic plans
    • Participate in the planning and implementation of operational issues
  • Coordinate ACD-CM and PMNWK strategic and business planning with stakeholders (e.g., Quality, School of Medicine, Stanford Advantage Plans)
  • Establish effective communication and relationships with providers in UHA, Stanford Medicine, and the community
  • Design and deliver education programs
  • Represent ACD-CM and PMNWK at medical group (UMP, Affinity, etc.) or UHA-wide meetings
  • Provide clinical direction for Accountable Care Division in collaboration with other Medical Directors and CMO Accountable Care
 
Program Design (estimated 10% of time)
  • Lead the development and design of the ACD-CM and the PMNWK programs
  • Develop the team and infrastructure
